Home IV Program

CLINICAL ORIENTATION OBJECTIVES:


GOAL/OBJECTIVE: I will be able to describe the common types of IV access devices that are avaliable at VGH and the differences between them.

HOW TO REACH/ACHIEVE IT: Through independent reading and asking questions to the Home IV pharmacists/ infusion nurses. Physically seeing the insertion and use of the individual devices will also be helpful with gaining a greater understanding of its place in therapy.

GOAL/OBJECTIVE: I will be able to describe the assessment process (i.e. criteria) of how a patient is admitted into the Home IV program and the successive process involved throughout the patient’s therapy.

HOW TO REACH/ACHIEVE IT: By following through with a patients from entry into the Home IV program until the end of his/her antibiotic therapy. By being involve with the patient’s drug management and regularly following up with the post-entry of the Home IV program.

GOAL/OBJECTIVE: I will be able to be familar with the different types of infections, contraindications, common adverse effects and monitoring parameters for antibiotics used by the Home IV patients.

HOW TO REACH/ACHIEVE IT: Through independent reading. Through practice by encountering different patient scenarios and being involved with the patient’s drug management process (i.e. drug monitoring). Through case discussions with the preceptor in charge.
